Flexible scopes with a fiber in the instrument channel are flexed up to 270 degrees in tight radius bends.  When laser light is slightly defocused, stray laser energy is allowed to launch into the fiber cladding and travels down the cladding and exists at the tight radius bend, burning a hole through the fiber cladding and the instrument channel of the ureteroscope!
