In urology, holmium laser fibers are passed through various types of ureteroscopes, both flexible and rigid, then laser energy is fired into the laser fiber. If the fiber is compromised, a small perforation in the instrument channel of a flexible scope will cause fluid to leak into the internal part of the scope and ruin the optics. Flexible scopes with a fiber in the instrument channel are flexed up to 270 degrees in tight radius bends. When laser light is slightly defocused, stray laser energy is allowed to launch into the fiber cladding and travels down the cladding and exists at the tight radius bend, burning a hole through the fiber cladding and the instrument channel of the ureteroscope!
